For private and NDIS participants - Home or community visits are avain Perth 30 minutes from Floreat (travel fees apply);

NDIS services
Self and Plan Managed Participants
NDIS services are available under Capacity Building - Improved Health and Wellbeing, Improved Daily Living or Early Childhood Intervention line items or potentially core funding - assistance with daily life. Please speak to your plan manager, support coordinator or LAC if you are unsure if you have funding for dietitian.
You will need to complete a service agreements to accept the terms of service before or during your first appointment. There is a 48 hours notice cancellation policy. Initial assessments are most often 90 minutes and reviews 30-60 minutes dependant on complexity.
Private services
No referrals required - full fee paying or Private Health Funds
Most Health funds provide rebates for dietitian. It's recommended to speak to your health fund prior to confirm rebates. All payments must be made in full then you can claim from your private health. HICAPS not available
Medicare services
Referral required from GP, Paedeatrician, Psychiatrist or specialist
Chronic Disease Management Plan (CDM) Adults
GPs can refer to dietitians for up to 5 sessions per year for chronic medical conditions - you can ask your GP to include autism or ADHD as a chronic neurodevelopmental condition which is affecting your nutrition intake. Medicare will rebate approximately $61.80 per session. Codes 10954, 93000, 93013
M10 Complex Neurodevelopmental Disorders < 25 yrs
Paedeatricians, Psychiatrists & other specialists can refer to dietitians for up to 10 treatment sessions per referral/20 per lifetime for neurodevelopmental Disorders with significant functional impairment or other eligible disability eg. Autism, ADHD, GDDCP, Fragile X, Down syndrome under this MBS M10 item. Medicare will rebate approximately $96.65 per session. Codes 82030, 82035, 93033, 93036, 93041, 93044
