Privacy Policy

1. What information we collect

Depending on the services you receive, we may collect:

  • your name and contact details;
  • date of birth and other identifying information;
  • information about your health, nutrition, disability, developmental history and medical history;
  • information about medications, allergies, diagnoses and relevant health conditions;
  • information about eating, feeding and nutritional concerns;
  • information provided by parents, carers or other authorised representatives;
  • information provided by other health professionals, support coordinators, carers, educators or other relevant people where you have provided appropriate consent or where permitted by law;
  • Medicare, NDIS and payment information where relevant;
  • appointment and service information;
  • information provided through our website, online booking system, enquiry forms, email, SMS or other communication methods; and
  • any other information reasonably necessary to provide safe and appropriate dietetic services.

Health information is sensitive information and we take particular care in handling it.


2. How we collect information

We may collect information:

  • directly from you;
  • from a parent, carer or authorised representative;
  • through our website and online booking forms;
  • during appointments;
  • through email, telephone, SMS or other communication;
  • from other health professionals or organisations where you have authorised us to obtain information or where disclosure is otherwise permitted by law; and
  • through third-party systems used to provide and administer our services.

Where practical, we will collect personal information directly from you or your authorised representative.


3. Why we collect and use your information

We collect and use personal information for purposes including:

  • providing dietetic and nutrition assessment, advice and support;
  • developing and reviewing nutrition plans and recommendations;
  • coordinating care with other health professionals and relevant supports where authorised;
  • communicating with you about appointments and services;
  • managing bookings and cancellations;
  • processing payments and invoices;
  • preparing reports and documentation where requested or required;
  • meeting NDIS, Medicare, taxation, legal and other regulatory requirements where applicable;
  • improving the safety and quality of our services;
  • responding to enquiries, feedback and complaints; and
  • managing our business operations.

We will only use or disclose your information for purposes permitted by law.


4. Disclosure of information

We may disclose personal information to third parties where reasonably necessary to provide or administer our services, where you have consented, or where disclosure is otherwise permitted or required by law.

Depending on the circumstances, this may include:

  • your parent, carer or authorised representative;
  • your GP or other treating health professionals;
  • occupational therapists, speech pathologists, psychologists or other allied health professionals;
  • support coordinators or other relevant NDIS supports;
  • plan managers or relevant NDIS parties where necessary for administration or payment;
  • Medicare or other government agencies where required;
  • payment providers;
  • booking, practice-management, accounting, communication and information-technology providers;
  • professional advisers, insurers or legal representatives where appropriate; and
  • government, regulatory or law-enforcement bodies where required or authorised by law.

We do not sell your personal information.


5. Third-party service providers

We use third-party technology and service providers including Splose and Heidi to help operate our practice. These may include services for appointment booking, client management, payment processing, accounting, electronic communication, document management and other business functions.

Where third-party providers handle personal information on our behalf, we take reasonable steps to select providers and arrangements that support the security and confidentiality of your information.

Some third-party providers may store or process information outside Australia. Where this occurs, we will take reasonable steps to comply with our obligations under the Privacy Act, including applicable requirements concerning overseas disclosure.


6. Storage and security

We take reasonable steps to protect personal information from misuse, interference, loss, unauthorised access, modification or disclosure.

Information may be stored electronically using secure practice-management, cloud-storage and other technology systems.

No electronic transmission or storage system can be guaranteed to be completely secure. We will respond appropriately if we become aware of a privacy or data-security incident.


7. Accessing and correcting your information

You generally have a right to request access to personal information we hold about you and to request that inaccurate or incomplete information be corrected.

We may need to verify your identity before providing access to information.

There may be circumstances where we are permitted by law to refuse access or correction. If this occurs, we will explain the reason where required.


8. Privacy complaints

If you believe we have mishandled your personal information, please contact us first so that we can investigate and respond to your concern.
