Terms & conditions

These Terms & Conditions apply to services provided by Neurodiversity Nutrition Pty Ltd as trustee for the Dutkowski Doering Family Trust ("Neurodiversity Nutrition", "we", "us" or "our").

By completing the intake form, booking or receiving services from Neurodiversity Nutrition, you agree to these Terms & Conditions and any applicable service agreement.

Neurodiversity Nutrition provides dietetic and nutrition services, which may include:

• nutrition assessment;
• nutrition education and advice;
• individualised nutrition support;
• neurodiversity-affirming nutrition support;
• feeding and mealtime support;
• parent and carer education and coaching;
• NDIS-related dietetic services;
• telehealth consultations;
• clinic appointments; and
• home or other agreed visits.

Services provided will depend on your individual circumstances and the service booked.

Our services are provided by qualified accredited practising dietitians within their scope of practice.

Nutrition recommendations are individualised and are based on the information available to us at the time of assessment.

Where appropriate, we may recommend that you seek assessment or treatment from another health professional or service.

Where services are provided to an NDIS participant, the services must be consistent with the participant's plan, applicable NDIS rules and pricing arrangements, and the agreed service agreement.

Neurodiversity Nutrition is an unregistered NDIS provider unless otherwise stated.

Unregistered providers can generally provide services to NDIS participants who are self-managing or plan-managed. Participants who are NDIA-managed generally need to use registered providers.

NDIS participants remain free to choose, change or leave providers in accordance with their NDIS arrangements.

Our fees will be communicated before or at the commencement of services and are available on our website.

Fees may vary depending on the type of appointment, location, travel, reports, administration or other services provided.

For NDIS services, fees will be consistent with the applicable NDIS requirements and the participant's agreed service arrangements.

Where NDIS pricing guidance or requirements apply, we will comply with the applicable requirements.

For private clients, payment is due according to the payment terms communicated at the time of booking or service.

Where a service involves travel, travel charges time and kilometres apply where agreed and where permitted under the relevant funding arrangements.

Any applicable travel charges will be communicated to you before the service where reasonably practicable.

Telehealth services may be offered where clinically appropriate.

You are responsible for ensuring that you have a suitable private environment and adequate internet or telephone access for the appointment.

If telehealth is not clinically appropriate or cannot be completed safely, we may recommend an alternative arrangement.

Our cancellation arrangements are set out in our Cancellation Policy and, for NDIS participants, in the applicable service agreement.

The cancellation policy is 48 hours notice with 100% of the fee. Where possible the time may be used on a non face to face activity where possible.

We may communicate with you by telephone, email, SMS or other agreed communication methods for purposes including appointment reminders, administration and service delivery.

Please let us know if you have communication preferences or accessibility requirements.

Reports, letters or other written documentation may incur a separate fee where this has been communicated and agreed.

We require reasonable notice to prepare documentation.

You may choose to stop receiving services from Neurodiversity Nutrition.

We may also end or pause services where appropriate, including where:

• the service is no longer clinically appropriate;
• we are unable to safely provide the service;
• the service falls outside our scope of practice;
• repeated non-attendance or cancellation makes ongoing service delivery impractical;
• there are unresolved payment issues; or
• another provider or service is more appropriate for your needs.

Where possible, we will discuss this with you and assist with an appropriate transition or referral.

Your personal and health information will be handled in accordance with our Privacy Policy.

We welcome feedback and complaints.

You can raise a concern directly with Neurodiversity Nutrition without fear of disadvantage or retaliation.

We may update these Terms & Conditions from time to time.

Where a change materially affects an existing service arrangement, we will communicate the change and seek agreement where required.
